Learner Enrolment and Support Needs Form

Purpose. Learning centre staff complete this form with each new learner during enrolment week to record who they are, what they want to learn, the support they need and what they hope to do next.
1.Completing this form
Sit with the learner and go through the form together. Read the questions aloud if the learner prefers. Explain any word the learner does not know. The learner signs the form, or makes a mark with a witness, when it is finished.
2.Privacy statement
The hub collects this information to enrol the learner, plan their learning and report to the Adult, Community and Further Education Board. It is stored in the locked office and in the Learner Register, is shared only as the funding agreement requires or as the learner agrees, and the learner can ask to see it at any time. Read this to the learner before they sign.
3.Support needs
Ask about anything that makes learning harder, such as eyesight, hearing, a disability, English as an additional language, caring responsibilities or transport. Record what the hub will do to help. This is not a test and no one is excluded for needing support.
4.Goals and next steps
Ask what the learner wants to be able to do at the end of the course and what job or study they are thinking about. Write it in the learner's own words. This becomes the first line of their individual learning plan.
5.After enrolment
Enter the learner in the Learner Register, file the form in the locked cabinet and book the learner's first class. Give the learner the welcome sheet with the timetable, the map and the coordinator's name.
